JPL Physical Therapy is an out-of-network physical therapy provider. I use the direct access model, in which I provide services without accepting insurances. I treat clients for intensive one hour sessions, one-on-one, for the most effective results to occur. I do encourage clients to contact their insurance carriers to inquire about what the out-of-network PT benefits consist of. Varying degrees of reimbursement for PT services can then be determined. I am happy to provide you with the necessary information you need to submit to your insurance company. This would include a “Superbill”, which lists the diagnosis codes and treatment codes provided during the session. You will also be given a payment receipt. You will provide an insurance claim submission form to your carrier.
